摘要:
糖尿病周围血管病变及血栓闭塞性脉管炎等疾病治疗难度大,主要原因为无法解决膝下动脉的“渐 进弥漫”闭塞导致腔内介入治疗中的动脉闭塞段无有效的流出道及动脉扩张后的通路无法维持的难题。本文总结 既往病例的影像学资料并结合实际经验,提出了对动脉闭塞部位实施持续脉冲式加压冲击治疗方案,以期达到以 下目的:利用高血流冲击压力保持已扩张的动脉通畅;利用加压血流的冲击使闭塞段逐步开放,并使导管逐步向 前推进;使“ 休眠”的侧枝动脉和微血管开放;直接促进闭塞部位动脉侧枝生长。如上述目的得以实现则为缓解 足部缺血提供了新的治疗思路。
Abstract:
It is difficult to treat the diseases such as diabetic perivascular disease (DPVD) and thromboangitis obliterans (TAO). The main reason is that the “progressive diffuse” occlusion of the inferior genicular artery can not be solved, which leads to the problems that there is no effective outflow tract and that the artery dilated pathway can not be maintained in the intracavitary interventional therapy. In this thesis, we summarized the imaging data of previous cases and combined them with our practical experience, and finally put forward a method of continuous pulse compression impact therapy for arterial occlusion. We plan to use high blood flow impact pressure to maintain the patency of dilated artery, open the occlusion segment gradually by the impact of pressurized blood flow, push the catheter forward step by step, open the “dormant” collateral arteries and microvessels, and promote directly the growth of the collateral branches of the occluded arteries. If these goals can be achieved, it will provide a new treatment for relieving foot ischemia.
